Dear Valued Patient,
I am writing to let you know that I have decided to close my practice, effective February 29, 2024. This has been an extremely difficult choice, as I have dedicated much of myself to this. I love providing neurologic care to you and my other patients; neurology is an area of medicine that can be scary and is often marginalized. I have appreciated the opportunity to step into that space and walk with you.
During my nearly 14 years in practice, I have had many life changes. My husband and I met and married – and we have adopted several children from foster care. The time has come for me to start a new chapter as a stay-at-home mom.
As for your care, my notes remain quite detailed and have been provided all along to your primary care physician, which should smooth your transition to another neurologist. I will be able to fill prescriptions and complete medication prior authorizations for you for about three months after your final visit.
You will need to work with your primary care to obtain a referral to a new neurologist. A records request form is included. This is for you to have your records transferred to your new neurologist or primary doctor. You will need to return this form in person at my office, by mail, or by fax. Requests will be fulfilled within 5 business days of receipt. Your doctor can also request your records by fax. All records requests must be in writing with a signature. Verbal requests cannot be fulfilled.
I have just two staff members who support me. Your understanding and cooperation regarding the amount of work we do for you and all my patients are requested.
